Too many times workers find a Band-Aid that stops the bleeding, but never go back and understand  why the problem happened in the first place.

Doing Triage is important, but getting to the final root cause is essential.

There are all sorts of ways to find possible causes to a problem, but unless you have someway to eliminate the causes, any and every possible cause must be Band Aided. Most people use a method of trial and error; try this, if the problem remains, then try that.

To complicate things more, workers leave all of the countermeasures on until something is decided to be the best possible cause. And countermeasures (Band-Aids) cost money due to the valuable time that workers need to dedicate to them.
